Hello, I’m Dr. Brian Cole and I’d like to provide you with an understanding of OrthoBiologics and how we use them in our clinical practice. We recommend two types of ortho biologic therapy. One is office based and the other is an adjunct to surgery. Platelet-rich plasma or PRP and bone marrow aspirate concentrate or BMAC, are options that are autologous-they come from you, the patient.
PRP is blood derived and bone marrow aspirate concentrate is bone marrow derived. It’s important to understand that they are both experimental and investigative in nature and they have an associated out-of-pocket expense that will not be covered by insurance.
PRP is a blood-derived product that is prepared in the office or during surgery. 20 cc’s of blood are drawn and centrifuged for five minutes and injected into the area of interest. PRP does not increase surgical time, prolong the recovery or require extra sutures.
BMAC, or bone marrow aspirate concentrate, is prepared intraoperatively while under anesthesia. 60 cc’s of bone marrow is extracted, centrifuged and injected into the area of interest. This also does not increase surgical time, prolong recovery or require extra sutures.
So why use PRP or bone marrow aspirate concentrate? Both PRP and BMAC have been found to improve symptoms or surgical outcomes in clinical trials and practice. The contents of these injections are believed to function much like biologic pharmacies delivering nutrients-growth factors in various biologic molecules to a site of degeneration, injury or surgical repair.
The mechanisms of action for these treatments are not fully delineated, but they are not necessarily related to stem cells nor are they considered regenerative in nature. Their potential benefits include reduced inflammation and pain, a positive impact on healing and improved surgical outcomes.
Some of the conditions that we commonly treat with these ortho biologics include Osteoarthritis, cartilage repair and transplantation, soft tissue repair such as repairing the rotator cuff, bone healing, ligament reconstruction and other surgical procedures.
